How to correct an L&I billing item that was denied due to name mismatch

Q: How should I correct an L&I billing item that was denied due to a claimant name mismatch?

A: You can go through CAC to obtain the official L&I version of the name (see screenshot below) and edit the Claimant First Name and Claimant Last Name in Gardiant Works.

Once the name is corrected, mark the L&I denied billing item for re-submission. The re-submitted item will reflect the correction in the name.

Note: The screenshot is of the “Health Insurance Claim Form” from the Provider Express Billing (PEB).
