[Admin report] OSHA Forms 300 and 300A

After validating that OSHA data is populated using the [Admin report] OSHA Claim Data Validation, use this report to populate OSHA forms 300 and 300A for an employer’s locations. Export to either Excel or PDF to print the forms. The 300A is formatted to print the report for each employer location on a separate page.

Individual claims listed in the 300 form are summed in the 300A form, which is the form that must be printed and posted in the workplace. Information must be added to this form to be considered complete (see Notes below).

Parameters:

  • Employer (required) allows you to narrow results to a specific employer account.

  • ^Let me pick inactive employers^ checkbox determines whether inactive accounts are included in the choices for selection in the Employer parameter. To see only active accounts, leave the checkbox unchecked (the default selection).

  • Employer Locations (optional) allows you to create reports for only selected employer locations. If left showing None selected, all employer locations will be included.

  • Calendar Year lets you specify which calendar year is being reported.

Notes:

  • The selected Employer must have employer locations for the report to run successfully (employer locations are created in the Locations sub-tab in an Account).

  • Incidents that don’t become claims in Gardiant cannot be included in the forms and may need to be added manually to the Excel export if the employer has recorded such incidents.

  • Some information needed to complete the OSHA 300A forms is not available in Gardiant and will need to be added manually to the Excel export. This includes SIC (Standard Industrial Classification) code, NAICS (North American Industrial Classification) code, annual average of employees for each employer location, and total hours worked by all employees last year for each employer location.
